There are a few places you can download and submit claim forms. Once you’ve logged in to your pet account:

• Under the Home tab, click the Download Claim Forms button to download the form(s) you need.
• Under the Claims tab, you can access the form(s) you need by clicking the Download Personalized Claim Form button.

To submit your claim forms:

• Under the Claims tab, you can also submit any claims by clicking the Submit Claim button.

Heads up! You can also download claim forms and submit them from the Quick links box located on the right side of your screen on every page.
